Attach recent passport-size photograph.
Prepare one set of attested photocopies of testimonials including:
MBBS/PG mark sheets and certificates
10th certificate for Date of Birth
Category certificate (if applicable)
Valid MCI/State Medical Council registration
Carry all original documents along with one set of self-attested copies.
Report to the Office of the Medical Superintendent, ESIC Hospital, Silvassa Road, Chanod, Vapi, Distt.- Valsad (Gujarat) on 20 January 2026.
Reach the venue between 9:00 AM to 11:00 AM for document submission and verification.
